Care planning

Overview

The Care Planning Engine combines what is known about a patient into an Integrated Care Plan — a set of available options, not a prescription.

The term is chosen carefully. A treatment plan is a physician’s professional medical decision. An integrated care plan assembles the evidence, the options, the providers and the costs, and presents them.

Inputs

  • Patient profile and reported symptoms
  • Health screening results and laboratory results
  • Measurements from appliances
  • Physician protocols and clinical guidelines
  • Insurance coverage and healthcare costs
  • Available healthcare providers

Workflow

The engine produces options. The patient reviews them and decides. Where the decision is medical, a physician confirms the final plan before it becomes a protocol.

Only then does the plan reach the YF1000 as something executable.

Why it matters

Keeping the decision with the patient and the physician is what allows the platform itself to sit outside the highest regulatory tier while still being clinically useful.
